Qualifications

• Minimum of 1-2 years of clinical nutrition experience (geriatrics, long-term care, or community health preferred).
• Minimum of one year working with the frail and elderly.
• Valid driver's license and reliable transportation.

Responsibilities

• Conduct comprehensive nutrition assessments upon enrollment.
• Develop and monitor individualized nutrition care plans.
• Provide medical nutrition therapy for chronic diseases.
• Provide individualized nutrition education and counseling to participants.

Skills

Clinical nutrition experience

Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN)

Experience with frail and elderly

ServSafe Food Protection Manager certification

Certified Diabetes Educator (CDE)

Education

Bachelor's or Master's degree in Nutrition, Dietetics, or related field
