Job summary

Brio Living Services in Grand Rapids, MI is seeking a Registered Dietitian to provide comprehensive nutrition care for residents in Porter Hills Village and Green House Homes. You will assess nutritional needs, develop and monitor individualized care plans, and collaborate with the interdisciplinary team to support health, safety, and quality of life.

The role requires an RDN, a bachelor’s degree in Dietetics or related field, and 3+ years of experience with frail elderly populations.

Qualifications

• Active RDN with CDR and AND credentials.
• Bachelor’s degree in Dietetics, Nutrition, Food and Nutrition or related field.
• Minimum 3 years’ experience with frail elderly in skilled nursing or senior living.
• Knowledge of geriatric nutrition, medical nutrition therapy and therapeutic diets.
• Experience with EMR and nutrition documentation.
• Strong communication, organization and time management; able to work with interdisciplinary team.
• Knowledge of CMS regulations, MDS and care planning.

Responsibilities

• Conduct comprehensive nutrition assessments for new admissions and at required intervals.
• Develop, implement and monitor individualized nutrition care plans.
• Identify and manage residents at nutritional risk.
• Monitor weights, intake, labs, and status; communicate changes to the team.
• Recommend and oversee therapeutic diets, supplements, hydration, and nutrition support.
• Collaborate with physicians, nursing, therapy and dining services for coordinated care.
• Participate in care plan meetings and quality improvement initiatives.
• Provide nutrition education to residents, families, caregivers, staff, and community.
• Review menus for nutritional adequacy and therapeutic diet alignment with culinary leadership.
• Conduct meal rounds and dining observations to identify improvements.
• Serve as resource regarding special nutrition needs to dining and culinary staff.
• Participate in resident and family meetings to support care and satisfaction.
• Support meal service and events as needed.
