Job summary

PACE of Southwest Michigan is seeking a Registered Dietitian to join the Participant Care team in Niles, MI. You will perform nutrition assessments, create individualized care plans, and provide medical nutrition therapy to support older adults’ health, independence, and quality of life.

This full-time, 40-hour weekly role offers a salary of $65,000–$75,000 per year plus a $5,000 sign-on bonus. Reports to Paula Shimulunas and collaborates with a multidisciplinary team across Niles and St.

Qualifications

• Bachelor’s or Master’s degree in Nutrition, Dietetics, or related field.
• Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential.
• 1–2 years clinical nutrition experience (geriatrics, long‑term care, or community health preferred).
• Minimum of one year working with the frail and elderly.
• Certified Diabetes Educator (CDE) desired.
• ServSafe Food Protection Manager certification required; if not held, obtained within six months of hire.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct comprehensive nutrition assessments at enrollment and at regular intervals.
• Develop, implement, and monitor individualized nutrition care plans.
• Provide medical nutrition therapy for chronic diseases.
• Monitor weight trends, labs, intake, and nutritional risk indicators.
• Recommend therapeutic diets, oral nutrition supplements, and texture-modified diets.
• Participate in IDT meetings, care planning and case conferences.
• Provide nutrition education and counseling to participants and caregivers.
• Conduct home visits to assess food access, safety, and meal preparation.
