Job summary

HopeWest in Grand Junction, CO seeks a registered dietitian to join our PACE team. You will assess nutritional needs and create individualized nutrition plans to support health, independence, and quality of life for older adults.

You will collaborate with physicians, nurses, therapists, and others to deliver participant-centered care, educate participants and families, and document according to PACE requirements.

Qualifications

• CDR registration is required to practice.
• ACEND-accredited supervised practice hours completed and NREx passed.
• Master's degree in dietetics or related field (or BA before 2024) and current CPR.

Responsibilities

• Assess nutritional status and needs of PACE participants.
• Develop, implement, and monitor nutrition care plans.
• Provide nutrition counseling to participants, families, and caregivers.
• Collaborate with physicians, nurses, therapists and other team members.
• Maintain accurate documentation per policies and PACE requirements.
