Job summary

Habitat Health in California is seeking a licensed Dietitian to join our multidisciplinary team serving older adults in PACE programs. You will assess nutritional status, develop care plans, educate participants and caregivers, and coordinate with the PCP and care team to optimize meals and supplements.

The role requires a Master’s degree in nutrition/dietetics, RD credential, and at least one year of experience with older adults.

Qualifications

Master's degree or equivalent in food science, nutrition, and/or dietetics as required by the credentialing body.
Nationally Certified as a Registered Dietitian Nutritionist.
Minimum of one year experience working with the frail or elderly.

Responsibilities

Develop and implement nutritional plan of care for participants in collaboration with the PCP and interdisciplinary team.
Plan and execute follow ups and reassessments for each participant.
Assess participants’ access to food and refer to interdisciplinary team for meal planning and groceries needs.
Order oral nutritional supplements as needed.
Provide dietary instruction to participants and caregivers.
Develop nutrition care plans for high-risk participants including medical nutrition therapy.
Monitor intake and dietary modifications and communicate with the care team.
