Qualifications

• Minimum of one year of experience working with a frail or elderly population required.
• Must have a valid driver's license and means of transportation.
• Medical clearance for communicable diseases and up-to-date immunizations required.

Responsibilities

• Screen and assess participant nutritional status for Senior Community Care participants.
• Plan and implement appropriate nutrition interventions and communicate them to the Interdisciplinary Team.
• Provide nutritional education for Senior Community Care participants requiring interventions.
