Qualifications

• Registered by the Commission on Dietetic Registration.

Responsabilités

• Assess residents' clinical nutrition needs and document the plan in the EMR.
• Develop, implement, and evaluate individualized care plans based on findings, labs, weight trends, intake and comorbidities
• Provide nutrition education and counseling to residents, families, and staff.
• Collaborate with interdisciplinary teams to determine a care plan that improves health and wellbeing.
• Ensure adherence to CMS, state, and federal regulations and company standards.
• Provide coverage in multiple buildings as needed and as requested.
• All other duties as assigned.

Formation

Registered Dietitian credential (CDR)

State license in good standing

RD-Eligible or provisional licensure
