Job summary

Healthcare Services Group (HCSG) is seeking a Registered Dietitian to manage clinical nutrition services at a long-term care facility in Kentucky. This on-site role emphasizes evidence-based nutrition plans, education for residents and staff, and collaboration with care teams to enhance resident health.

You will assess nutrition needs, document plans in the EMR, and ensure CMS/state/federal compliance while providing counseling to residents and their families.

Qualifications

• Registered Dietitian Nutritionist required
• Experience with clinical nutrition in long-term care
• Strong communication and organizational skills
• Knowledge of CMS/state/federal regulations preferred
• RD must be in good standing in state of practice

Responsibilities

• Assess residents' nutrition needs and document plans in the EMR
• Develop, implement, and evaluate individualized care plans based on clinical findings and weight trends
• Provide nutrition education and counseling to residents, families, and staff
• Collaborate with interdisciplinary teams to improve residents' health
• Ensure adherence to CMS, state and federal regulations and company standards
• Provide coverage in multiple buildings as needed
• Perform all other duties as assigned
