Job summary

Healthcare Services Group, Inc is hiring a Registered Dietitian to manage clinical nutrition services at a long-term care facility in Albany, KY. This role supports resident health, adheres to CMS/state guidelines, and offers Monday–Friday schedules with flexible options to meet site needs.

You will assess residents’ nutrition needs, develop and document care plans, provide education to residents, families and staff, and collaborate with interdisciplinary teams.

Qualifications

• Registered by the Commission on Dietetic Registration.
• Certification/License in good standing in the state of practice.
• Knowledge of state survey process and nutrition compliance (preferred).
• Strong organizational and communication skills.
• Maintain a positive attitude and communicate with staff and leadership.
• Compliance with COVID-19 vaccination policies as required.
• If multi-site, must have a valid driver's license.
• RD-Eligible and provisional licensure accepted in certain locations.

Responsibilities

• Assess residents' clinical nutrition needs and document the plan in the EMR.
• Develop, implement, and evaluate individualized care plans based on findings.
• Provide nutrition education and counseling to residents, families and staff.
• Collaborate with interdisciplinary teams to improve health outcomes.
• Ensure CMS, State and Federal regulations and company standards are followed.
• Provide coverage in multiple buildings as needed.
• All other duties as assigned.
