You will assess residents, develop individualized care plans, and provide counseling, while ensuring compliance with regulations and collaborating with interdisciplinary teams.

Qualifications

• Registered by the Commission on Dietetic Registration.
• Licensure in good standing within the state of practice.
• Knowledge of state survey processes and compliance preferred.

Responsibilities

• Assess clinical nutrition needs of residents and document plans in the EMR.
• Develop, implement, and evaluate individualized care plans based on findings and weight trends.
• Provide nutrition education and counseling to residents, families, and staff.
• Collaborate with interdisciplinary teams to improve health outcomes.
• Ensure adherence to CMS, state, and federal regulations and company standards.
• Provide coverage across multiple buildings as needed.
