Healthcare Services Group (HCSG) is hiring a Registered Dietitian to manage clinical nutritional services at a long-term care facility. You will assess residents, develop and document care plans within the electronic medical record, and provide nutrition education to residents, families, and staff.

The role requires licensure in good standing, RD certification, strong organizational and communication skills, and ability to collaborate with interdisciplinary teams while ensuring CMS and state

Competenze

• Registered by the Commission on Dietetic Registration.
• State licensure in good standing where practicing.
• Knowledge of CMS/state nutrition care regulations (preferred).
• Strong organizational and communication skills.

Mansioni

• Assess residents' clinical nutrition needs and document plans in the EMR.
• Develop, implement, and evaluate individualized care plans based on clinical data.
• Provide nutrition education and counseling to residents, families, and staff.
• Collaborate with interdisciplinary teams to improve health outcomes.
• Ensure adherence to CMS, state, and company standards and regulations.
• Provide coverage across buildings as needed.
• All other duties as assigned.
