Qualifications

• RD credential and license required in state of practice
• Knowledge of nutrition regulations; state/federal compliance preferred

Responsibilities

• Assess clinical nutrition needs of residents/patients and document plan in EMR
• Develop, implement and evaluate individualized care plans based on findings
• Provide nutrition education and counseling to residents/patients, families and staff
• Collaborate with interdisciplinary teams to improve health outcomes
• Ensure adherence to CMS, State and Federal regulations and company standards
• Provide coverage in multiple buildings as needed
• All other duties as assigned
