Job summary

Senior Community Care of Michigan PACE is hiring a Registered Dietitian in Lansing, MI to deliver personalized nutrition care to seniors in a PACE setting. The role emphasizes evidence-based practice and collaboration with the interdisciplinary care team to support overall participant well-being.

Your responsibilities include conducting nutrition assessments, planning interventions, and monitoring outcomes while coordinating with caregivers and staff across shifts.

Qualifications

Bachelor’s or advanced degree in nutrition or dietetics from an accredited college.
Licensed in the state as a Dietitian; Registered Dietitian Certification required.
Minimum of one year of experience with a frail or elderly population.
Must have a valid driver’s license and transportation.

Responsibilities

Provide Nutrition Care using the Academy of Nutrition and Dietetics framework and Medical Nutrition Therapy.
Conduct annual in-person re-assessments and follow-ups per Plan of Care.
Collaborate with the Interdisciplinary Team to coordinate 24-hour care delivery.
Monitor weights, diagnoses, hydration, and changes affecting nutrition and care plans.
