Senior Community Care of Michigan PACE in Lansing, MI is seeking a part-time Registered Dietitian to join its on-site team. The RD will travel locally to participant homes and provide nutrition therapy within a healthcare setting, aligning with the PACE program mission.

The role requires licensure in Michigan, RD certification, and experience with frail or elderly populations. Competitive pay range, 8:00 a.m.–4:30 p.m. schedule, 2–3 days weekly.

Qualifikationen

• Bachelor's or advanced degree in Dietetics required.
• Licensed in the state as a Dietitian; Registered Dietitian Certification required.
• Minimum of one year of experience working with a frail or elderly population.
• Must have a valid driver's license and transportation.

Aufgaben

• Provides quality Nutrition Care through the Nutrition Care Process; delivers Medical Nutrition Therapy to participants and/or caregivers.
• Conducts in-person re-assessments on schedule and when status changes.
• Collaborates with Interdisciplinary Team for care planning and 24/7 care delivery coordination.
• Monitors weights, diagnoses, drug-nutrient interactions, hydration, and intake; reports changes to the team.
