Job summary

Napa Post Acute in Napa, CA is seeking a Registered Dietitian (RD) to join our interdisciplinary healthcare team. This full-time position offers $50-$55 per hour and the opportunity to support resident health, wellness, and recovery through personalized nutrition care.

You will conduct nutrition assessments, develop care plans, monitor status, and collaborate with physicians, nursing, speech therapists, and dietary staff to ensure optimal resident outcomes while complying with regulations.

Qualifications

• Current RD/RDN credential required.
• Licensed or eligible for CA licensure.
• Experience in skilled nursing, post-acute care, rehabilitation, or healthcare preferred.
• Strong knowledge of clinical nutrition, therapeutic diets, and regulatory requirements.
• Excellent communication, organizational, and documentation skills.

Responsibilities

What You'll Do

• Conduct comprehensive nutrition assessments for residents upon admission and as needed


• Develop, implement, and monitor individualized nutrition care plans


• Evaluate nutritional status and provide recommendations to support resident health and recovery


• Collaborate with physicians, nursing staff, speech therapists, and dietary personnel to ensure optimal resident outcomes


• Monitor residents for significant weight changes, nutritional risks, and dietary concernsParticipate in care plan meetings and interdisciplinary team discussions


• Ensure compliance with state, federal, and facility regulations regarding nutrition services


• Provide nutrition education and counseling to residents, families, and staff as needed


• Support the Dietary Services department in maintaining high-quality nutrition programs

Qualifications

• Current Registered Dietitian (RD/RDN) credential required


• Licensed or eligible for licensure in California, as applicable


• Experience in skilled nursing, post-acute care, rehabilitation, long-term care, or healthcare preferred


• Strong knowledge of clinical nutrition, therapeutic diets, and regulatory requirements


• Excellent communication, organizational, and documentation skills


• Ability to work independently while collaborating effectively with an interdisciplinary team
