Job summary

Pacific Coast Post-Acute is seeking a knowledgeable, compassionate Registered Dietitian to oversee the nutritional care of residents in our 149-bed skilled nursing facility. You will assess, develop, implement, and monitor individualized nutrition plans as part of an interdisciplinary team to promote recovery and quality of life.

The ideal candidate has experience with skilled nursing regulations, geriatrics, and a passion for improving resident well-being.

Qualifications

  • Strong knowledge of skilled nursing and long-term care nutrition regulations.
  • Experience with geriatric nutrition and quality improvement initiatives.
  • 1–3 years of experience in acute care, skilled nursing, or long-term care.

Responsibilities

  • Complete comprehensive nutritional assessments and identify residents at nutritional risk.
  • Develop, implement, and monitor individualized nutrition care plans.
  • Monitor weight trends, labs, intake, and overall nutritional status.
  • Recommend therapeutic diets, supplements, and nutrition interventions.
  • Provide nutrition education and counseling to residents, families, and staff.
  • Maintain accurate clinical documentation.

Job description

Pacific Coast Post-Acute is seeking a knowledgeable, compassionate, and dedicated Registered Dietitian (RD) to join our team. In this role, you will oversee the nutritional care of residents in our 149-bed skilled nursing facility , helping promote optimal health, recovery, and quality of life through individualized nutrition care.

Compensation

Pay Range: $45.00-$60.00 per hour , depending on experience and qualifications.

Position Summary

The Registered Dietitian is responsible for assessing, developing, implementing, and monitoring individualized nutrition plans for residents. You will work closely with nursing, dietary services, physicians, and other members of the interdisciplinary team to ensure residents receive appropriate, high-quality nutritional care.

The ideal candidate is knowledgeable about skilled nursing regulations, understands the unique nutritional needs of older adults, and is passionate about making a positive difference in residents' lives.

Key Responsibilities
  • Complete comprehensive nutritional assessments and identify residents at nutritional risk
  • Develop, implement, and monitor individualized nutrition care plans
  • Monitor residents' weight trends, laboratory values, food intake, and overall nutritional status
  • Make appropriate recommendations for therapeutic diets, supplements, and nutrition interventions
  • Provide guidance regarding food-drug interactions and other nutrition-related concerns
  • Collaborate with nursing, dietary services, physicians, and the interdisciplinary care team
  • Participate in care planning and resident-centered quality improvement initiatives
  • Monitor and support compliance with federal, state, and local regulations related to nutrition and dietary services
  • Provide nutrition education and counseling to residents, families, and staff as appropriate
  • Support resident satisfaction and contribute to a positive dining experience
  • Maintain accurate and timely clinical documentation
Requirements
  • Strong knowledge of skilled nursing and long-term care nutrition regulations and compliance standards
  • Understanding of the aging process and the nutritional needs of long-term care residents
  • Excellent communication, organization, and critical-thinking skills
  • Ability to build positive relationships with residents, families, and staff
  • Ability to work independently while collaborating effectively with an interdisciplinary team
Preferred Qualifications
  • Bachelor's degree in Dietetics, Nutrition, or a related field from an accredited institution
  • Completion of an accredited dietetics education and supervised practice program
  • 1-3 years of experience in acute care, skilled nursing, or long-term care
  • Experience with PointClickCare (PCC) and/or US Foods systems
  • Previous experience working with geriatric populations is a plus
