Job summary

Sweetwater Care seeks a Therapy Resource to lead therapy services across multiple skilled nursing and long-term care facilities. You will partner with Administrators and Directors of Rehabilitation to drive clinical excellence and optimize operations.

You will monitor performance metrics, coach DORs, and oversee education, program development, and staffing strategies while traveling to facilities as needed.

Qualifications

• Licensed professional in PT, OT, or SLP with active license.
• 3–5 years of experience in skilled nursing or long-term care.
• Director of Rehabilitation experience preferred.
• Strong understanding of SNF regulatory requirements and reimbursement models.
• Ability to analyze performance metrics and drive improvements.

Responsibilities

• Monitor therapy performance indicators across assigned facilities.
• Coach Directors of Rehabilitation on financial and operational performance.
• Support budget adherence and forecasting for therapy services.
• Lead education and implementation of clinical programs.
• Collaborate with Administrators, DORs, and regional leaders to align goals.
• Travel up to ~50% for on-site support, audits, and training.
