Job summary

Enhabit Home Health & Hospice in Tyler, TX is seeking a Regional Referral Coordinator to ensure smooth transitions to home health services. You will work with referral sources and branch staff to coordinate assessments and admissions, applying current Medicare guidelines and regulatory standards.

This role requires a high school diploma, at least one year in home health, a valid driver license, CPR certification if licensed, and proficiency with Microsoft Office.

Qualifications

• High school diploma or equivalent required.
• At least 1 year of home health experience.
• Knowledge of Medicare guidelines governing home health.
• Valid state driver’s license in state of residence.
• Automobile liability insurance as required by law.
• Graduate of an approved school of clinical education (nursing or therapy) preferred.
• If licensed, maintain valid clinical license and CPR certification.
• Homecare Homebase (HCHB) experience preferred.
• Strong oral/written communication in English; active listening.
• Proficient Microsoft Office (Word, Excel).
• In-office attendance required to support supervision and teamwork.

Responsibilities

• Coordinate regional patient referrals and transitions to home health services.
• Act as a resource for referral sources and collaborate with branch staff.
• Ensure adherence to Medicare guidelines governing home health admissions.
• Represent the branch in interactions with referral sources.
• Maintain up-to-date knowledge of regulatory guidelines for home health admissions.
• Support coordination efforts to achieve Enhabit objectives.
