Job summary

Enhabit Home Health & Hospice is seeking a Referral Coordination Specialist to oversee regional patient referrals and collaborate with branch staff to ensure smooth transitions to home health services.

The role requires knowledge of Medicare guidelines, strong communication skills, and full in-office presence to support supervision, teamwork, and task completion. Prior home health experience and familiarity with HCHB are advantageous.

Qualifications

• Oral and written communication in English is essential.
• Proficiency with Microsoft Word and Excel required.
• Experience in home health and knowledge of Medicare guidelines is important.
• Valid driver license and insurance as per state requirements.
• CPR certification and clinical license where applicable is preferred.

Responsibilities

• Responsible for regional coordination of patient referrals and acts as a resource for referral sources.
• Works closely with branch staff to ensure timely coordination and transition to home health services.
• Demonstrates knowledge of regulatory guidelines governing home health admissions and represents the branch in discussions with referral sources.
