Job summary

NorthStar Care Community is seeking a Regional Practice Manager to lead daily palliative care operations, guiding Nurse Practitioners, RNs, and Medical Assistants toward high-quality, patient-centered care. You will design workflows, foster engagement, and collaborate with hospice teams and community partners to ensure regional alignment and KPI achievement.

Responsibilities include managing budgets, staffing, and scheduling, driving performance via data-driven insights, and sustaining a culture

Qualifications

• Master’s degree in healthcare administration or equivalent leadership experience.
• Minimum of 3 years in healthcare operations, practice management, hospice, palliative care, or related clinical setting.
• Experience supervising clinical or administrative staff preferred.

Responsibilities

• Lead day-to-day practice operations for the palliative care team and achieve clinical and financial KPIs.
• Build strong relationships with hospice teams, referral partners, providers, and leadership.
• Monitor staffing, productivity, and resource utilization to ensure efficient, high-quality care.
• Oversee budgeting, scheduling, and workflow processes to support timely visits and efficient operations.
• Promote continuous improvement through process optimization and stakeholder collaboration.
