Sweetwater Care in NorCal (Stockton and Sacramento) seeks an outstanding MDS nurse consultant to provide expertise, leadership, and support for organization-wide initiatives. You will implement and evaluate the facility’s Quality Measurement and Performance Improvement program to ensure quality of resident care outcomes.

• Must possess a current, active license to practice as a registered nurse in California.
• Minimum 3 years of RN experience, with supervisory experience preferred.
• Experience with MDS completion and process.
• Resident Assessment Coordinator (RAC) Certification preferred.
• Understanding of computer technology, including EHR systems.

Responsabilidades

• Oversee facilities’ comprehensive resident assessment process per state and federal regulations.
• Audit completion of MDS assessments and supporting documentation.
• Review medical records for MDS documentation; provide education as needed.
• Interpret rules and guidelines; act as resource for care-based reimbursement and quality reporting.
• Provide consultative training to MDS Coordinators to coordinate resident-centered care.
• Assist Interdisciplinary Team with Care Planning.
• Guide documentation and coding related to RAI.
• Support facilities to achieve 5-Star Quality Measure ratings.
• Audit and monitor RAI timeliness and accuracy to regulatory standards.
• Identify trends, develop action plans, and perform root-cause analyses.
• Review data to ensure accurate diagnosis capture per PDPM guidelines.
• Provide ongoing education and support to optimize quality of care delivery.
• Attend and support Quality Assurance Performance Improvement Programs (QAPI).
• Advocate for MDS Coordinators with facility leadership and IDT.
• Maintain up-to-date knowledge of state and federal RAI and reimbursement guidelines.
• The Regional MDS Nurse assists facilities in surveys and is available during survey periods.
