Feather River Care Center in NorCal (Stockton & Sacramento) seeks an experienced MDS nurse to provide expertise, leadership, and support for organization-wide initiatives and the implementation and evaluation of the facility’s Quality Measurement and Performance Improvement program to ensure high-quality resident care outcomes.

The Regional MDS Nurse will audit MDS documentation, train MDS Coordinators, and assist with surveys and 5-Star quality measures.

Qualifications

  • Current, active RN license to practice in the state.
  • 3 years of RN experience with supervisory exposure preferred.
  • Experience with MDS completion and process.
  • RAC Certification preferred.

Responsibilities

  • Oversee resident assessment process per state/regulations.
  • Audit MDS completions and supporting docs.
  • Review records for MDS coding accuracy; provide education.
  • Interpret rules and support care-based reimbursement and quality reporting.
  • Train MDS Coordinators and coordinate resident-centered care.
  • Assist Interdisciplinary Team with care planning.
  • Guide documentation related to RAI and PDPM.
  • Support facilities to achieve 5-Star Quality Measure ratings.
  • Audit RAI timeliness and accuracy for regulatory standards.
  • Identify trends, develop action plans, and perform root-cause analysis.
  • Prepare facilities for surveys and assist during surveys.

Position Description

We are looking for an outstanding MDS nurse to work closely with our facilities in NorCal- Stockton and Sacramento. This consultant will be responsible for providing expertise, leadership, modeling, and support for organization-wide initiatives. He/she will also work on the implementation and or evaluation of the facility’s Quality Measurement and Performance Improvement program to ensure quality of resident care outcomes.

Qualifications
  • Must possess a current, active license to practice as a registered nurse in this state.
  • 3 years’ experience as a registered nurse, one of which includes supervisory experience, preferred.
  • Experience with MDS completion and process.
  • Resident Assessment Coordinator (RAC) Certification is preferred.
  • Understanding of computer technology, including Electronic Health Record (EHR) systems
Duties and Responsibilities
  • Overseeing assigned facilities’ comprehensive resident assessment process in accordance with state and federal regulations.
  • Auditing completion of MDS assessments and any supporting assessments or clinical documentation.
  • Reviewing medical records for the presence of supporting documentation for all items coded on the MDS. Provide education as necessary.
  • Interpreting rules, regulations and coverage guidelines and acting as primary resource for problem solving regarding care-based reimbursement systems and quality reporting program.
  • Provide consultative support and training to MDS Coordinators within the assigned facilities, to coordinate and guide resident-centered care.
  • Assist the Interdisciplinary Team with Care Planning.
  • Provide additional guidance related to clinical documentation and coding as it relates to the Resident Assessment Instrument (RAI).
  • Provide support and guidance to assist assigned facilities to achieve and maintain 5-Star Quality Measure Rating.
  • Auditing and monitoring of RAI timeliness and accuracy of information in furtherance of regulatory standards.
  • Support resident care by identifying trends and developing processes and action plans. Must be able to perform and present Root-Cause-Analysis.
  • Reviews and audits data to support assigned facilities’ obligation to accurately capture diagnosis codes in accordance with PDPM guidelines.
  • Provide on-going education, support, and make recommendations to optimize quality of care delivery.
  • Attend and support assigned facilities’ Quality Assurance Performance Improvement Programs (QAPI).
  • Work as an advocate for MDS Coordinators in relationship to the facility leadership, and the Interdisciplinary Team (IDT).
  • Maintain the most current knowledge of State and Federal guidelines and regulations related to the RAI and reimbursement.
  • The Regional MDS Nurse will assist the facility to prepare for the surveys and should be available to assist during the survey.
