Job summary

Almond View Care Center seeks an experienced MDS Nurse consultant to support NorCal facilities in Stockton and Sacramento. You will lead the resident assessment process, ensure MDS accuracy, and drive quality initiatives across the organization.

Responsibilities include auditing MDS completion, training MDS Coordinators, supporting QAPI, and helping facilities achieve 5-Star quality measures while staying current with PDPM guidelines.

Qualifications

• Active RN license in the state.
• Minimum 3 years as RN with supervisory experience preferred.
• Experience with MDS completion and process.
• RAC Certification preferred.
• Familiarity with Electronic Health Record (EHR) systems.

Responsibilities

• Oversee facilities’ resident assessment process per state/federal regulations.
• Audit MDS completions and supporting assessments or documentation.
• Review medical records for MDS codes; provide education as needed.
• Interpret rules/regulations and support care-based reimbursement/quality reporting.
• Provide consultative training to MDS Coordinators for resident-centered care.
• Assist Interdisciplinary Team with Care Planning.
• Guide documentation and coding related to the RAI.
• Support facilities to achieve and maintain 5-Star Quality Measure Rating.
• Audit RAI timeliness/accuracy to meet regulatory standards.
• Identify trends, develop action plans, perform root-cause analysis.
• Review data to ensure proper diagnosis codes per PDPM guidelines.
• Provide ongoing education and support to optimize care delivery.
• Attend/support Quality Assurance Performance Improvement Programs (QAPI).
• Advocate for MDS Coordinators with facility leadership/IDT.
• Stay current with State/Federal RAI and reimbursement guidelines.
• Assist during surveys and be available to support during survey periods.

Position Description

We are looking for an outstanding MDS nurse to work closely with our facilities in NorCal- Stockton and Sacramento. This consultant will be responsible for providing expertise, leadership, modeling, and support for organization-wide initiatives. He/she will also work on the implementation and or evaluation of the facility’s Quality Measurement and Performance Improvement program to ensure quality of resident care outcomes.

Qualifications

• Must possess a current, active license to practice as a registered nurse in this state.
• 3 years’ experience as a registered nurse, one of which includes supervisory experience, preferred.
• Experience with MDS completion and process.
• Resident Assessment Coordinator (RAC) Certification is preferred.
• Understanding of computer technology, including Electronic Health Record (EHR) systems

Duties and Responsibilities

• Overseeing assigned facilities’ comprehensive resident assessment process in accordance with state and federal regulations.
• Auditing completion of MDS assessments and any supporting assessments or clinical documentation.
• Reviewing medical records for the presence of supporting documentation for all items coded on the MDS. Provide education as necessary.
• Interpreting rules, regulations and coverage guidelines and acting as primary resource for problem solving regarding care-based reimbursement systems and quality reporting program.
• Provide consultative support and training to MDS Coordinators within the assigned facilities, to coordinate and guide resident-centered care.
• Assist the Interdisciplinary Team with Care Planning.
• Provide additional guidance related to clinical documentation and coding as it relates to the Resident Assessment Instrument (RAI).
• Provide support and guidance to assist assigned facilities to achieve and maintain 5-Star Quality Measure Rating.
• Auditing and monitoring of RAI timeliness and accuracy of information in furtherance of regulatory standards.
• Support resident care by identifying trends and developing processes and action plans. Must be able to perform and present Root-Cause-Analysis.
• Reviews and audits data to support assigned facilities’ obligation to accurately capture diagnosis codes in accordance with PDPM guidelines.
• Provide on-going education, support, and make recommendations to optimize quality of care delivery.
• Attend and support assigned facilities’ Quality Assurance Performance Improvement Programs (QAPI).
• Work as an advocate for MDS Coordinators in relationship to the facility leadership, and the Interdisciplinary Team (IDT).
• Maintain the most current knowledge of State and Federal guidelines and regulations related to the RAI and reimbursement.
• The Regional MDS Nurse will assist the facility to prepare for the surveys and should be available to assist during the survey.
