Job summary

Sweetwater Care is seeking an MDS Nurse Consultant to support NorCal facilities in Stockton and Sacramento. You will lead resident assessment activities, ensure MDS accuracy, and guide care planning across the Interdisciplinary Team.

The role requires an active RN license in California, 3+ years of RN experience with supervisory exposure, and MDS expertise. RAC certification is preferred, with strong EHR familiarity and regulatory knowledge.

Qualifications

• Active RN license in the state of California.
• Minimum 3 years as an RN, with at least some supervisory experience.
• Experience with MDS completion and process.
• RAC Certification is preferred.
• Familiarity with Electronic Health Records (EHR) systems.

Responsibilities

• Oversee facilities' resident assessment process per state and federal regs.
• Audit MDS assessments and supporting docs for accuracy.
• Review medical records for MDS coding justification and educate as needed.
• Interpret rules, regs, and coverage guidelines; advise on care-based reimbursement and quality reporting.
• Provide consultative training to MDS Coordinators to guide resident-centered care.
• Assist Interdisciplinary Team with care planning.
• Support QA/QI programs (QAPI) and regulatory surveys.
• Advise facilities to achieve and maintain 5-Star Quality Measure Rating.
• Present Root-Cause-Analysis findings and develop improvement plans.
• Maintain updated knowledge of RAI, PDPM guidelines, and related regulations.
