Job summary

ChenMed in Philadelphia seeks an Intensive Community Care Manager (ICCM), an RN leading complex care for high-risk patients. You will coordinate care plans with PCPs, conduct home visits, and supervise team members to keep patients safely at home and engaged in care.

Requires active RN license, 2+ years clinical experience, strong communication, and ability to travel regionally. On-site role with day hours, supporting West and North Philadelphia offices.

Qualifications

• RN license in state of employment (compact preferred).
• Associate degree in Nursing required; BSN preferred.
• Minimum 2 years of clinical experience; case management experience desired.

Responsibilities

• Provide in-house, facility, and telephonic visits to high-risk patients to prevent hospitalizations.
• Perform home visits, assess patient, develop and implement care plans with PCP collaboration.
• Supervise LPN and staff education; ensure safe discharge from case management where applicable.
• Coordinate, document, and follow up on multidisciplinary team meetings and patient care plans.
