Avantara in Rapid City, SD is hiring a Hospital Liaison to oversee regional business development and drive census growth across Avantara locations. The role emphasizes building partnerships with hospital discharge planners and ensuring smooth transitions for residents.

The ideal candidate will be an LPN or RN with 2 years in long-term care, capable of strategic planning, relationship-building, and cross-functional collaboration with facility leadership and local hospital networks.

Qualifications

• Hold a current, unencumbered LPN or RN nursing license.
• Experience in hospital or long-term care settings.
• Strong relationship-building and communication skills.
• Ability to travel regionally to support multiple facilities.

Responsabilités

• Cultivate and maintain relationships with referral sources across your region.
• Promote Avantara’s post-acute care services to healthcare providers, hospitals, and community organizations.
• Work with facility leadership to optimize admissions processes and enhance resident and family satisfaction.
• Provide clinical onsite coordination with local hospital networks to ensure continuity of care during transitions.
• Develop and execute strategic plans to increase census growth based on data-driven insights and market trends.
• Represent Avantara at networking events, community engagements, and healthcare expos to strengthen brand presence.
• Monitor referral pipelines and provide regular reports on activity, conversions, and growth metrics.
