Qualifications

Responsibilities
- Cultivate and maintain relationships with referral sources across the assigned region.
- Promote post-acute care services to healthcare providers, hospitals, and community organizations.
- Collaborate with facility leadership to optimize admissions and resident satisfaction.
- Provide clinical onsite coordination with local hospital networks during transitions.
- Develop and execute plans to increase census growth based on data and market trends.
- Represent Avantara at networking events and healthcare expos to strengthen brand presence.
- Monitor referral pipelines and report activity and growth metrics.
