University Clinical Health, Inc. is seeking a Regional Health Coach Manager to lead Health Hubs in an assigned Tennessee region, combining leadership with direct health coaching to empower patients in high-need communities.

You will supervise Health Coaches, coordinate multi-site operations, perform health assessments, deliver wellness and chronic disease self-management programs, and ensure HIPAA-compliant Epic EHR documentation while building partnerships with local providers.

Qualifications

• Must complete Motivational Interviewing and Health Coaching certifications within 45 days of employment.
• Must complete National Diabetes Prevention Program (DPP) and Diabetes Self-Management Education (DSME) certifications within the first 12 months.
• 3 years of progressive work experience in healthcare.
• 2-4 years of supervisory or management experience.
• Strong interpersonal and verbal/written communication skills.

Responsabilités

• Provide operational leadership for Health Hubs within an assigned region.
• Supervise Health Coaches and support multi-site operations.
• Conduct health assessments and deliver wellness/self-management programs.
• Maintain Epic EHR documentation and HIPAA compliance.
• Build community partnerships and manage referrals with providers and local organizations.
