Job summary

The Agency for Persons with Disabilities is seeking an OPS Regional Residential Planner to process residential referrals for group home placement and coordinate with internal and external stakeholders.

You will chair weekly residential planning meetings, schedule coordination calls, contact crisis stabilization units and hospitals for discharge planning, and maintain residential tracking systems to ensure seamless service delivery.

Qualifications

• Valid Driver's License or reliable transportation
• High School Diploma or equivalent
• Minimum 2 years experience working with individuals with disabilities or similar human services
• Experience planning service needs for individuals with developmental disabilities
• Proficient with Microsoft Word, Excel, SharePoint and PowerPoint
• Ability to communicate effectively orally and in writing with diverse stakeholders
