Job summary

Danville-Pittsylvania Community Services in Virginia seeks a regional geriatric and dementia services expert to lead coordination for initiatives increasing access to quality care and step-down services.

As a project manager, you will partner with CSB staff and community providers to advocate for community-based care in the least restrictive settings and ensure compliance with reporting requirements.

Qualifications

• Bachelor’s degree in a human services field or an equivalent Virginia nursing license.
• Valid driver’s license with a safe driving record.
• Fingerprints, state and FBI background checks, drug test, and CANIS registry as required.

Responsibilities

• Serve as regional subject matter expert on geriatric individuals and dementia.
• Provide project management and coordination for regional initiatives to improve access to quality services.
• Collaborate with CSB staff and community providers, advocating community-based care in the least restrictive settings.

Skills

Valid driver's license

Education

Bachelor’s Degree in Psychology, Social Work, Vocational Rehabilitation, Counseling, or other approved Human Services degree

Virginia Nursing License (LPN or RN)
