Danville-Pittsylvania Community Services in Virginia seeks a regional geriatric and dementia services expert to lead coordination for initiatives increasing access to quality care and step-down services.
As a project manager, you will partner with CSB staff and community providers to advocate for community-based care in the least restrictive settings and ensure compliance with reporting requirements.
Provides support to the Department of Behavioral Health and Developmental Services (DBHDS), the Community Services Boards (CSB) of Region 3b of Virginia, and the community at large by serving as the regional subject matter expert on geriatric individuals and those living with dementia; provides project management and coordination for the development and implementation of regional initiatives related to increasing access to and obtaining quality services for geriatric and individuals with dementia related diagnoses at risk for hospitalization and/or in need of stepdown services from state hospitalization; partners with CSB staff and acts as liaison to community providers, advocating community-based care in the least restrictive and appropriate care settings.

Annual Salary Range: $44,825 - $77,617
