Job summary

Saber Healthcare Group in Centerville, VA is seeking a dedicated therapy professional to coordinate dementia programming within our skilled nursing facility. You will collaborate with the interdisciplinary team to design and implement compassionate, evidence-based activities that support residents’ cognitive, emotional, and physical well‑being.

The role requires COTA (preferred) or related degree, Dementia Practitioner certification (or within 6 months), and at least 2 years’ dementia or memory

Qualifications

• COTA preferred; may consider related field degree.
• Current Dementia Practitioner certification preferred or obtainable within six months.
• Minimum of two years' experience in long-term care or memory care programming.

Responsibilities

• Coordinate with interdisciplinary team to provide dementia-focused programming.
• Plan, organize, implement, and oversee specialized dementia activities.
• Support residents’ emotional, cognitive, physical and social well-being through structured programming.
