Job summary

Bravia Services LLC is seeking a Regional Care Specialist to conduct assessments and provide intake counselling for Medicaid Waiver and LTSS participants in Indiana. You will ensure timely, accurate assessments and streamlined access to support services, working with internal teams and external providers.

The role requires RN or health/social work degree with relevant human services experience, strong communication, and proficiency with Microsoft Office and InterRAI systems.

Qualifications

• Requires RN or health/social work degree or nursing degree; relevant human services experience preferred.
• Ability to assess needs and provide accurate counselling regarding options.
• Proficient in Microsoft Word/Outlook/Excel and able to use digital tools.

Responsibilities

• Conduct assessments and provide intake counselling for Medicaid Waiver and LTSS participants.
• Ensure assessments and counselling comply with state directives, processes and timeframes.
• Collaborate with internal teams and external providers to connect clients to services.
