CWS Durham is seeking a Refugee Youth Mentoring Case Manager to support refugee and immigrant youth and families through mentoring, case management, and connections to community resources that promote school success and social integration.

The role requires trauma-informed, culturally responsive care, strong documentation and advocacy skills, and the ability to coordinate with partners and service providers. Hybrid or in-office work is possible.

Qualifications

• Bachelor's degree or equivalent experience in social work or related field.
• Minimum 3 years of case management experience.
• Trauma-informed, culturally responsive care knowledge.
• Excellent communication and documentation skills.
• Ability to manage diverse caseloads under pressure.
• Familiarity with local social services resources.
• Proficiency with Microsoft 365 and case management software.

Responsabilités

• Conduct intake assessments to determine client needs, risks, and eligibility for services.
• Develop individualized service plans with measurable goals and timelines.
• Provide ongoing case management, monitoring progress, and adjusting plans as needed.
• Facilitate access to housing, healthcare, employment, education, legal support, and financial assistance.
• Advocate for clients with external agencies to secure necessary resources.
• Maintain accurate, timely, and confidential case files and track outcomes.
• Participate in trainings to maintain best practices in trauma-informed care and client advocacy.
• Perform other duties as assigned.
