Church World Service, Inc. is seeking a Refugee Youth Mentoring Case Manager to support refugee and immigrant youth and families through one-on-one and group mentoring, case management, and connection to resources that promote school success and social integration.

You will conduct needs assessments, develop service plans, coordinate referrals, maintain confidential files, and advocate for clients within community systems, ensuring trauma-informed, culturally responsive care.

Formación

• Bachelor's degree or equivalent experience in social work, human services, psychology, public health, or related field.
• Minimum 3 years of case management or direct service experience, preferably in a nonprofit or humanitarian setting.
• Strong understanding of trauma-informed, strength-based, and culturally responsive care.
• Excellent communication, documentation, and interpersonal skills.
• Ability to manage diverse caseloads and work effectively under pressure.
• Knowledge of local community resources and social service systems.
• Proficiency in Microsoft 365 and case management software.

Responsabilidades

• Conduct intake assessments to determine client needs, risks, and eligibility for services.
• Develop individualized service plans with measurable goals and timelines.
• Provide ongoing case management, monitoring progress, and adjusting plans as needed.
• Facilitate access to housing, healthcare, employment, education, legal support, and financial assistance.
• Offer crisis intervention and safety planning as appropriate.
• Advocate for clients with external agencies to secure necessary resources.
• Maintain accurate, timely, and confidential case files.
• Track client outcomes and prepare reports for internal use and grant compliance.
• Contribute to program development, improvement, and evaluation.
• Support outreach, workshops, and community education activities.
