Job summary

Lutheran Family Services Rocky Mountains is seeking a Family Stabilization Specialist to provide crisis mental-health case management for refugees and related at-risk families in Colorado. You’ll collaborate with resettlement staff and clients to create Plans of Care and connect them with essential services.

The role requires cultural sensitivity, strong organizational skills, and the ability to navigate complex family dynamics while documenting case activity accurately and timely.

Qualifications

  • Master's degree in social work or related field with at least one year of crisis case management experience.
  • Experience with refugees or immigrants prioritized; ability to manage high-risk situations.
  • Ability to access and coordinate community resources and services.

Responsibilities

  • Advocate for refugee clients and ensure equal treatment across client groups.
  • Develop individualized Plans of Care with a collaborative team approach.
  • Identify and screen client needs to arrange appropriate services.
  • Coordinate referrals to mental health, housing, benefits, and employment resources.
  • Provide psychoeducational support and crisis navigation, updating Plans of Care.
  • Deliver trauma-informed, family-centered interventions and culture-informed practices.
  • Facilitate translation/interpreting services when needed.
  • Build client confidence through coaching and problem solving.
  • Develop partnerships to expand access to community resources.
  • Maintain detailed case notes and timely documentation.
  • Contribute to staff trainings on refugee mental health and wellness topics.
  • Submit required program reports timely and accurately.
  • Comply with agency, state, and federal standards and contracts.
