Job summary

The Lutheran Family Services Rocky Mountains is seeking a Case Manager to support foster care youth and families across Colorado’s Front Range. You will monitor safety, coordinate services, and advocate for resources within a trauma-informed framework.

You will manage an assigned caseload, complete documentation, participate in court and school meetings, and collaborate with placement providers. Bilingual candidates are preferred and a valid driver’s license is required.

Qualifications

• Familiarity with assessment, care planning, facilitation, coordination, and advocacy for support on behalf of and in collaboration with individuals or families.
• Knowledge of community resources and ability to connect youth or families to appropriate resources through referral or enrollment assistance.
• Skills and sensitivity to work with children, youth and families from diverse ethnic and cultural backgrounds.
• Familiarity with social work practice, human development, and the child welfare system, including local, state and federal regulations.

Responsibilities

• Manage all assigned cases with face-to-face contact with youth in placement per state regulations.
• Coordinate monitoring of therapeutic, health, educational and cross-cultural resources for youth and families.
• Generate case notes and maintain documentation to meet timeliness and accuracy standards.
• Provide on-call services for emergencies and crises.
• Participate in court hearings, placement review meetings, and school meetings.
