Job summary

AltaMed Health Services Corp. seeks a Referral Coordinator to ensure timely access to specialty care by accurately entering referral requests and tracking follow-ups. You will act as a bridge between patients, providers, and Medical Management to streamline the care pathway.

The role requires a high school diploma, at least 4 years of experience, and preferred Medical Assistant certification. Bilingual abilities in English, Spanish, Mandarin, or Cantonese are valued.

Job Overview

The Referral Coordinator is responsible for accurate and timely data entry of referral requests into the appropriate portal.This position is also responsible for follow-up on all specialty referrals.This position is an integral part of the care team, working as a liaison between the patient, provider, and Medical Management to ensure timely access to specialty care.

Minimum Requirements

• High school diploma required.
• Minimum of 4 years of experience required.
• Medical Assistant certificate preferred
• Bilingual English/Spanish/Mandarin/Cantonese preferred.

Compensation

$26.78 - $31.47 hourly
