Job summary

AltaMed Health Services in Santa Ana, CA is seeking a Clinic Referral Coordinator to ensure timely access to specialty care. You will enter referral requests into the correct portal and follow up on all referrals, coordinating care for urgent cases as part of the care team.

In this role, you will act as a liaison among patients, providers, utilization management, and various departments while maintaining accurate data and timely communication.

Qualifications

• High school diploma required.
• Minimum of 4 years of experience required.
• Medical Assistant certificate preferred.
• Bilingual English/Spanish/Mandarin/Cantonese preferred.
• A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

Responsibilities

Job Overview

The Referral Coordinator position is responsible for accurate and timely data entry of referral requests into the appropriate portal. They are also responsible for follow-up on all specialty referrals and coordination of care for urgent referrals. The referral coordinator is an integral part of the care team, working as a liaison between the patient, providers, utilization management, and various departments to ensure timely access to specialty care.

Minimum Requirements

• High school diploma required.

Compensation

$25.00 - $29.32 hourly
