Hospice Austin is seeking a Referral Coordinator to support the referral and admissions process for hospice and palliative care services. The role includes conducting informational visits with patients, families, and referral sources, and explaining eligibility and support options.

You will perform palliative care social work visits, complete documentation, and coordinate with the care team to ensure a compassionate transition into services.

Formación

• Master's degree in social work required or equivalent.
• Licensed Clinical Social Worker (LCSW) license required.
• Experience in hospice or palliative care preferred.

DescriptionJOB TITLE: Referral CoordinatorDEPARTMENT : Access and MarketingPOSITION SUMMARYThe Referral Coordinator is a licensed social worker responsible for supporting the referral and admissions process for Hospice Austin and Austin Palliative Care. This position conducts informational visits with patients, families, caregivers, and referral sources to explain hospice and palliative care services, eligibility requirements, and available support. The Referral Coordinator also provides assigned palliative care social work visits, completes timely documentation, coordinates with members of the interdisciplinary team, and helps ensure a compassionate and seamless transition into services.WORKING RELATIONSHIPSREPORTS TO : Director of Admissions, Access and Palliative CareSUPERVISES : NoneINTERPERSONAL RELATIONSHIPS : All comments, actions and behaviors have a direct effect on the public’s perception of Hospice Austin. Interactions with patients, family members, physicians, referral sources, visitors, co-workers, etc. must be in a manner that is courteous, respectful, cooperative and professional.
