Qualifications

• Experience working with patient referrals and intake processes.
• Experience verifying insurance and coverage (Medicaid/Medicare).
• Proficiency with Microsoft Office (Excel, Word, Outlook).
• Hospice/healthcare experience preferred.
• Detail oriented and highly organized.
• Effective written and verbal communication.
• Clean background and drug screen completed.

Responsibilities

• Process referrals to ensure timely follow-up on diagnoses.
• Verify insurance coverage as part of the intake process.
• Coordinate patient intake with providers and care teams.
• Maintain accurate patient records and documentation.
