Job summary

Hospice Austin is seeking an RN Admissions professional to evaluate and admit patients into hospice care, ensuring eligibility and understanding of services. You will perform comprehensive clinical assessments and coordinate with physicians and the interdisciplinary team to complete required documentation accurately and timely.

You will also act as the primary contact for patients, families, and referral sources, providing education on hospice philosophy, goals of care, and end-of-life support,

Qualifications

  • Graduation from an approved school of professional nursing required.
  • Licensed as a Registered Nurse by the Texas Board of Nurse Examiners required.
  • Minimum of 3 years of professional nursing experience, with at least 1 year in leadership or mentorship.
  • CPR certification required.
  • Knowledge of hospice regulations preferred.

POSITION SUMMARY

Responsible for evaluating and admitting patients into hospice care, ensuring they meet eligibility criteria and understand the services provided. This role involves conducting comprehensive clinical assessments, coordinating with physicians and the interdisciplinary team, and completing all required documentation accurately and timely.

Serves as a key point of contact for patients, families, and referral sources—providing education on hospice philosophy, goals of care, and end-of-life support. The AdmissionsNurse also facilitates a smooth transition into hospice by initiating care plans and ensuring all services are in place in different care settings.

WORKING RELATIONSHIPS

REPORTS TO: Team Leader

SUPERVISES: None

INTERPERSONAL RELATIONSHIPS:

All comments, actions and behaviors have a direct effect on the public’s perception of Hospice Austin. Interactions with patients, family members, physicians, referral sources, visitors, co-workers, etc. must be in a manner that is courteous, respectful, cooperative and professional. This behavior should promote an atmosphere of teamwork and positive relations.

ESSENTIAL FUNCTIONS
  • Assesses patients for hospice eligibility, coordinates admissions, and ensures timely, accurate documentation.
  • Educate patients and families on hospice care, collaborate with the care team, and help ensure a smooth transition into services with a strong focus on compassionate, patient-centered care.
Qualifications:
  • Graduation from an approved school of professional nursing required.
  • Licensed as a Registered Nurse by the Texas Board of Nurse Examiners required.
  • Minimum of 3 years of professional nursing experience, with at least 1 year in a leadership or mentorship role.
  • Valid Texas driver’s license, auto liability insurance, and a safe driving record required.
  • CPR certification required.
  • Previous hospice or palliative care experience strongly preferred.
  • Valid Texas driver’s license, auto liability insurance and safe driving record. (if required for the position)
  • Knowledge of hospice federal and state regulations preferred.
  • Strong computer skills including knowledge of and experience with Microsoft Office products and experience with electronic medical records (EMR).
PREFERRED SKILLS:
  • Adhere to company policies regarding attendance, safety, and mandatory training.
  • Promote the mission and goals of Hospice Austin through quality leadership and adaptability.
  • Participate in and lead performance improvement initiatives to enhance patient care and team effectiveness.
