The Care Coordinator is a highly visible, patient-focused role that works directly with our patient population, their families, insurance representatives, outside vendors, physicians, clinicians, and other medical personnel in a dynamic environment to provide high-quality healthcare to our members.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
  1. Coordinate and process patient referrals accurately and efficiently.
  2. Prioritize high-priority patients, especially those in Primary Care Physicians' panels, and expedite referrals as needed.
  3. Schedule patients with specialists and notify them of appointment details.
  4. Use Web IVR systems (Availity, Careplus, Healthhelp NIA, etc.) to generate authorizations.
  5. Document all referral activities clearly, including scheduling and notification details.
  6. Send referrals to specialists and confirm receipt.
  7. Participate in daily huddles using RITS Huddle Portal, communicating needs and updates.
  8. Enter procedures into the HITS system.
  9. Follow up on missed appointments and reschedule as necessary.
  10. Address referral-related calls promptly.
  11. Complete patient checkouts, assisting as needed.
  12. Retrieve consultation notes and follow up on home health and DME orders.
  13. Provide exceptional customer service to all internal and external customers.
  14. Utilize patient messaging tools effectively.
  15. Perform additional duties as assigned.
KNOWLEDGE, SKILLS, AND ABILITIES:
  • Bilingual in English and Spanish is a plus but not required.
  • Detail-oriented with strong multi-tasking skills.
  • Proficient in navigating computer systems and software.
  • Excellent communication skills and professional phone etiquette.
  • Team-oriented with the ability to work well with diverse groups.
  • Knowledge of medical terminology, CPT, HCPCS, and ICD coding is preferred.
EDUCATION AND EXPERIENCE CRITERIA:
  • High School diploma or equivalent required.
  • At least 1 year of referral experience in healthcare.
  • Experience with web-based insurance sites and obtaining referrals/authorizations is preferred.
  • Experience with Medicare Advantage populations and medical assistant certification are pluses.
