Job description

Role Overview

The Care Coordinator is a highly visible customer service and patient-focused role. The incumbent works directly with our patient population and their families, insurance representatives, outside vendors, physicians, clinicians, and other medical personnel to ensure the referral process runs smoothly. Operating in a dynamic and professional environment, this role ensures the highest level of quality healthcare is delivered to our members.

Essential Job Duties/Responsibilities
  1. Coordinates and processes patient referrals to completion with precision, detail, and accuracy.
  2. Prioritizes HPP patients in Primary Care Physicians panel, expedites and orders over 5 days.
  3. Ensures orders are approved when needed and schedules patients with specialists, notifying them of appointment details.
  4. Uses Web IVR to generate authorizations and completes orders with proper documentation.
  5. Sends referrals to specialists and confirms receipt.
  6. Prepares and participates in physician/clinician daily huddles, communicating needs and outstanding items.
  7. Enters all inpatient and outpatient elective procedures in the HITS tool.
  8. Reschedules and communicates external missed appointments.
  9. Participates in Super Huddle, providing updates on high-priority referrals.
  10. Addresses referral-based phone calls and completes messages within 24 hours.
  11. Checks out patients based on their assigned physician/clinician, assisting colleagues if unavailable.
  12. Retrieves consultation notes and follows up on Home Health and DME orders.
  13. Provides extraordinary customer service to all internal and external customers, utilizing patient messaging tools.
  14. Performs other related duties as assigned.
Knowledge, Skills, and Abilities
  • Knowledge of medical terminology, CPT, HCPCS, and ICD coding desired.
  • Detail-oriented with multi-tasking ability and openness to cross-training.
  • Proficient in phone etiquette and navigating computer systems.
  • Team-oriented with excellent interpersonal skills.
  • Exceptional organizational skills and proficiency in Microsoft Office Suite.
  • Willingness to travel locally up to 10%.
  • Fluent in English; bilingual is a plus.
Education and Experience
  • High School diploma or equivalent required.
  • Minimum of 1 year of healthcare referral experience required.
  • Experience with web-based insurance sites, Web IVRs, and Medicare populations preferred.
  • Medical Assistant certification and CPR certification are preferred.
