Job description

The Care Coordinator is a highly visible, patient-focused role. This position works directly with patients, their families, insurance representatives, outside vendors, physicians, clinicians, and other medical personnel to ensure the referral process runs smoothly. The environment is dynamic and professional, aiming to deliver the highest quality healthcare to our members.

ESSENTIAL JOB DUTIES/RESPONSIBILITIES:
  1. Coordinate and process patient referrals with precision, detail, and accuracy.

Definition of completion:

  1. Prioritize HPP patients in the Primary Care Physicians panel, expedite, and order over 5 days.
  2. Ensure orders are approved when needed.
  3. Schedule patients with providers from the Preferred Providers List and notify them of appointment details.
  4. Use Web IVR systems (Availity, Careplus, Healthhelp NIA, etc.) for authorizations.
  5. Complete orders with proper documentation of scheduling and notifications.
  6. Send referrals to specialists and confirm receipt.
  7. Participate in physician/clinician daily huddles using RITS Huddle Portal, communicating needs or outstanding items.
  8. Enter all inpatient and outpatient elective procedures in HITS.
  9. Reschedule and communicate external missed appointments to physicians/clinicians.
  10. Participate in Super Huddle, providing updates on high-priority referrals.
  11. Handle referral-related phone calls for the Primary Care Physicians panel.
  12. Respond to phone messages within 24 hours.
  13. Check out patients based on their assigned physician/clinician, assisting colleagues if unavailable.
  14. Retrieve consultation notes from the consult tracking tool.
  15. Follow up on Home Health and DME orders to ensure service delivery.
  16. Provide exceptional customer service to all internal and external customers, including patients and team members, utilizing messaging tools.
  17. Perform other duties as assigned.
KNOWLEDGE, SKILLS, AND ABILITIES:
  • Knowledge of medical terminology, CPT, HCPCS, and ICD coding is desirable.
  • Detail-oriented with multi-tasking ability and willingness to cross-train.
  • Proficient in phone etiquette and navigating computer systems.
  • Team-oriented, professional, and courteous in interactions.
  • Excellent organizational skills with effective prioritization.
  • Proficient in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and database management.
  • Willing to travel locally up to 10%.
  • Fluent in English; bilingual skills are a plus.
EDUCATION AND EXPERIENCE CRITERIA:
  • High School diploma or equivalent required.
  • At least 1 year of referral experience in healthcare required.
  • Experience with web-based insurance sites and obtaining referrals/authorizations preferred.
  • Experience with Web IVRs and Medicare Advantage population preferred.
  • Medical Assistant certification and CPR certification are preferred.
