Qualifications

The Care Coordinator is a highly visible, patient-focused role. You will work directly with patients, families, insurance reps, vendors, physicians, and clinicians to ensure smooth referral processes, delivering high-quality healthcare.

ESSENTIAL JOB DUTIES/RESPONSIBILITIES:
  1. Coordinate and process patient referrals accurately and efficiently.
  2. Prioritize HPP patients, expedite and order referrals over 5 days, with approved orders when needed.
  3. Schedule patients with specialists and notify them of appointment details.
  4. Use Web IVR systems for authorizations and document all actions properly.
  5. Confirm receipt of referrals sent to specialists.
  6. Participate in daily huddles, communicate needs, and update on high-priority referrals.
  7. Enter procedures into the HITS system and reschedule missed appointments.
  8. Follow up on home health and DME orders to ensure service delivery.
  9. Provide exceptional customer service to all internal and external customers, utilizing messaging tools.
  10. Perform other duties as assigned.
Knowledge, Skills, and Abilities:
  1. Knowledge of medical terminology, CPT, HCPCS, and ICD coding preferred.
  2. Detail-oriented with multitasking ability; cross-training willingness.
  3. Proficient in phone etiquette and computer software systems.
  4. Team-oriented, professional, and courteous.
  5. Excellent organizational skills with effective prioritization.
  6. Proficient in Microsoft Office Suite and database software.
  7. Willingness to travel locally up to 10%.
  8. Fluent in English; bilingual is a plus.
Education and Experience:
  1. High School diploma or equivalent required.
  2. At least 1 year of healthcare referral experience required.
  3. Experience with web-based insurance sites and authorization processes preferred.
  4. Experience with Medicare Advantage and medical assistant certification is a plus.
