Job summary

East Valley Community Health Center is seeking a Referral Coordinator to ensure patients access specialty services promptly. You will coordinate referrals, enter authorizations, and communicate with providers and patients in a fast-paced clinical setting.

The role requires Spanish proficiency, experience with NextGen and EHR, and the ability to work with the health care team. DOE salary range $22.00–$25.98 per hour, with strong benefits.

Qualifications

• Two years of clinical experience preferred.
• Spanish proficiency for effective communication with customers.
• Knowledge of referral procedures preferred.
• Knowledge of NextGen, EHRs, and i2i preferred.
• Strong interpersonal and verbal/written communication skills.
• Proficient with Microsoft Word and Excel for data entry.

Responsibilities

• Enter referral authorization requests into IPA and County systems.
• Coordinate and follow up on referrals for specialties or diagnostics.
• Communicate with providers and patients; prioritize urgent referrals.
• Maintain electronic logs of referrals in the i2i Population Health system.
• Enter relevant medical information into NextGen order management.
• Mail out authorization letters and follow up with patients.
• Attend meetings offsite/onsite/Zoom and collaborate with the care team.
• Ensure confidentiality of sensitive information.

Job description

The Referral Coordinator will ensure that patients needing specialty services can access them promptly and efficiently. The emphasis is on the ongoing interactions of team members to discuss roles, responsibilities, communication, and patient hands-off, working together to provide and enhance the care provided to patients.

MAJOR POSITION RESPONSIBILITIES AND FUNCTIONS:

• Enter referral authorization requests into both the IPA and County electronic systems.
• Coordinate and follow up on referrals for specialties or diagnostics.
• Communicate with providers and patients.
• Demonstrate ability to prioritize and handle urgent referrals.
• Maintain electronic logs of all referrals in the i2i Population Health system
• Enter pertinent medical information into the order management section in NextGen.
• Enter referral authorization requests into both the IPA and County electronic system.
• Obtain consultation reports.
• Attend meetings offsite/onsite/zoom.
• Work cooperatively with all members of the healthcare team.
• Work with the health care team to ensure prompt processing of referrals.
• Mail out authorization letters.
• Follow up with patients as needed or as requested.
• Answer incoming calls, and respond to voice messages.
• Attending assigned training either offsite or onsite.
• Respond promptly and appropriately to patient and/or provider requests.
• Demonstrate dignity with kindness and compassion to all patients, visitors, and staff.
• Scan all referral requests, authorizations, and consult notes into the patient’s electronic medical record.
• Maintain confidentiality of sensitive information
• Other responsibilities as assigned.

POSITION REQUIREMENTS AND QUALIFICATIONS:

• Knowledge of referral procedures is preferred.
• Knowledge of and experience with NextGen and EHR preferred.
• Knowledge of i2i
• Two years of experience in a clinical setting is preferred.
• Must be proficient in Spanish to effectively communicate with customers
• Excellent interpersonal skills to care for patients continually
  Excellent Communication skills both verbal and written.
• Proficient in Microsoft Word, Excel, and Data Entry
• Able to work with minimal supervision.
• Able to work in a fast-paced environment.

DOE $22.00 - $25.98
