Job summary

MyCHN seeks a dedicated Referral Case Manager to connect patients with specialty services, coordinate care, and maintain clear communication among providers and patients. The role emphasizes accuracy, timeliness, and patient-centered support.

In this fast-paced environment, you will educate patients, assist with barriers to care, and collaborate with internal teams to ensure seamless care transitions and compliant documentation in the EHR.

Responsibilities

  • Monitor and process referrals daily.
  • Review referral information for accuracy and necessity.
  • Coordinate specialty referrals per guidelines and provider availability.
  • Educate patients on referral status and scheduling steps.
  • Assist patients with barriers to specialty care (transport, insurance, scheduling).
  • Communicate with providers and care teams to ensure continuity of care.
  • Document referral activities in the EHR and track outcomes.
  • Escalate urgent cases to clinical leadership as needed.
  • Support quality initiatives and HIPAA compliance.
  • Participate in team meetings and workflow improvements.

As a Referral Case Manager, you'll play a vital role in connecting patients with specialty services, coordinating care, and ensuring seamless communication between providers, specialists, and patients.

Your Responsibilities
  • Monitor and process referrals generated by MyCHN providers on a daily basis.
  • Review referral information for accuracy, completeness, and medical necessity.
  • Coordinate specialty referrals according to established guidelines and provider availability.
  • Educate patients on referral status, appointment scheduling, and next steps in their care journey.
  • Assist patients in overcoming barriers to specialty care, including transportation, insurance verification, and appointment coordination.
  • Communicate effectively with providers, specialists, and internal clinical teams to ensure continuity of care.
  • Maintain accurate documentation of referral activities and patient interactions within the Electronic Health Record (EHR).
  • Track referral outcomes and ensure appropriate follow-up.
  • Escalate urgent or complex cases to clinical leadership when necessary.
  • Support quality initiatives and maintain compliance with organizational policies and HIPAA regulations.
  • Participate in team meetings and contribute to workflow improvement initiatives.
What You'll Need
  • High School Diploma or equivalent

  • Proficiency with Electronic Medical Records (EMR) systems and basic computer applications

Preferred Qualifications
  • Previous experience in healthcare administration or a similar healthcare setting
  • Medical Assistant Certification
