Job summary

Cano Health is seeking a Referral Coordinator to support patient care coordination by managing insurance pre-verifications, referrals, and scheduling. You will liaise with patients, providers, and health plans to ensure timely referrals and accurate authorizations.

The role emphasizes patient-centered service, collaboration with care teams, and compliance with HIPAA and data privacy. Clinical experience with EHRs and a bilingual English/Spanish skill set is preferred.

Qualifications

• High school diploma or GED required.
• Minimum 1–2 years handling referrals in a medical office, hospital, or clinical setting.
• Working knowledge of medical terminology and insurance processes.
• Experience with scheduling systems and EHRs (eClinicalWorks).
• BLS certification required; must respond to emergencies.

Responsibilities

• Process outgoing referrals promptly based on provider orders.
• Review, validate, and document referral orders in data systems.
• Schedule appointments with in-network specialists, prioritizing cost-effective options.
• Track referral status and follow up on incomplete referrals.
• Close the referral loop by routing reports to appropriate providers.
• Educate patients on the referral process and insurance requirements.
• Assist patients with barriers (transportation, scheduling, language).
• Verify insurance eligibility and referral requirements; obtain authorizations as needed.
• Collaborate with payers, specialists, and care teams to expedite approvals.
• Maintain accurate documentation of all referral activities in the EHR system.
• Log referral status and communications in a timely manner.
• Work with physicians, nurses, and care managers to coordinate care.
• Uphold HIPAA guidelines and clinic data privacy.
