Job summary

Anchorage Neighborhood Health Center is seeking an RN Care Manager in Alaska to provide intensive, relationship-based care coordination for medically and socially complex patients at high risk for ED visits and hospitalization.

You will identify high-need patients, enroll them in care management, and coordinate across primary care, behavioral health, and community services to improve engagement and outcomes.

Qualifications

• Four years of progressively responsible nursing experience in primary care, ambulatory care, community health, or population health setting.
• Active RN license in Alaska with current BLS certification.
• Bilingual skills preferred.

Responsibilities

• Develop individualized, patient-centered care plans across care settings.
